Evaluate your pet's interest in food and ability to eat.
1 = No interest in food
3 = Reduced appetite
5 = Normal appetite
3. HYDRATION - Water Intake
Consider your pet's drinking habits and hydration status.
1 = Dehydrated/not drinking
3 = Drinking less than normal
5 = Normal water intake
4. HYGIENE - Grooming & Cleanliness
Assess your pet's ability to maintain personal hygiene.
1 = Cannot groom, soiling self
3 = Some grooming issues
5 = Good hygiene habits
5. HAPPINESS - Joy & Responsiveness
Evaluate your pet's emotional state and interest in surroundings.
1 = No interest, withdrawn
3 = Some response to family
5 = Alert, engaged, joyful
6. MOBILITY - Movement & Activity
Consider your pet's ability to move around and perform daily activities.
1 = Cannot stand/walk
3 = Limited mobility
5 = Normal movement
7. MORE GOOD DAYS THAN BAD
Overall assessment of your pet's daily experience.
1 = Mostly bad days
3 = Equal good and bad
5 = Mostly good days
