1. HURT - Pain Management

Consider your pet's pain level and how well it's being managed.

Pain Level:
1 = Severe pain 3 = Moderate pain 5 = No apparent pain

2. HUNGER - Appetite & Eating

Evaluate your pet's interest in food and ability to eat.

Appetite:
1 = No interest in food 3 = Reduced appetite 5 = Normal appetite

3. HYDRATION - Water Intake

Consider your pet's drinking habits and hydration status.

Hydration:
1 = Dehydrated/not drinking 3 = Drinking less than normal 5 = Normal water intake

4. HYGIENE - Grooming & Cleanliness

Assess your pet's ability to maintain personal hygiene.

Hygiene:
1 = Cannot groom, soiling self 3 = Some grooming issues 5 = Good hygiene habits

5. HAPPINESS - Joy & Responsiveness

Evaluate your pet's emotional state and interest in surroundings.

Happiness:
1 = No interest, withdrawn 3 = Some response to family 5 = Alert, engaged, joyful

6. MOBILITY - Movement & Activity

Consider your pet's ability to move around and perform daily activities.

Mobility:
1 = Cannot stand/walk 3 = Limited mobility 5 = Normal movement

7. MORE GOOD DAYS THAN BAD

Overall assessment of your pet's daily experience.

Good vs Bad Days:
1 = Mostly bad days 3 = Equal good and bad 5 = Mostly good days

Additional Considerations
